Communication breakdowns, hierarchical limitations, and staff exhaustion are leading causes of the insignificant rise in medical errors worldwide. Donaldson et al. (2000) revealed medication errors occur in 7 to 9% of prescriptions, leading to 1% of patient harm. Transformational leadership can address these healthcare challenges through its dynamic and people-oriented approach for enhanced patient safety outcomes. This study is a systematic review examining the literature on how leadership practices directly impact care coordination, staff empowerment, and communication practices in healthcare settings, as well as how transformational leadership enables the development of a proactive environment supporting teamwork while encouraging responsibility and continuous learning to enhance patient safety. The findings included key themes such as establishing open communication channels, team empowerment, and collaboration, as fundamental to decreasing medical errors and enhancing patient results. These demonstrate leadership’s necessity to promote trust, innovation, and accountability within healthcare organizations. Future research would benefit from quantitative surveys to establish statistical relationships between leadership approaches and patient results while performing qualitative interviews to identify healthcare professional insights regarding transformational leadership’s effects on team functioning and care delivery quality.
